Well it depends on the enemy and also the difficulty setting, I assume. Even on normal difficulty, certain boss type enemies like the phylakes and those big mfkers in the forts can't be instantly killed, even when they may be at your level or two levels below, but the hidden blade will still deal significant damage.

Bayek lost his ring finger using the blade the first time, Altair, Ezio, and all the others had their primary blade on their left forearm. I have always taken this since Altair to be symbolic of being married to the creed as he didn't have his left ring finger.
